I can ride out a mild case of PI with topical stuff. When I managed to get it right next to my eye, I was swiftly put on prednisone. I got a bad all-over-arms-and-legs case in high school and was absolutely put on prednisone. One of my compatriots on that adventure was out of school for more than two weeks, so I know it can be very bad for at least some people.
