  I think a road test for *everyone* (whatever age) every five years would probably be a good idea, but that would cost more than any state would ever add to a budget.
  In December, my 86 year old mother's NC license was renewed with only a vision test. She has self-imposed limitations (she doesn't drive at night, or in precipitation, or on highways), but I don't think the state put restrictions on her other than eyeglasses.

I also think the MA written test study guide should be more complete. Until a couple of years ago,  I did not know that it is against the law to do a U-turn across a double yellow line.  I had always assumed that it was only where there were "No U-turn" signs.  Police officers know - why don't we?
